Failing to assess risks 'might have serious consequences' The warning was issued by the Health and Safety Executive (HSE) after RT Trailers and Farm Buildings was fined £50,000 plus costs for a workplace accident in which an employee lost a leg.

David Farr was loading steel panels onto a tractor and grain trailer, when he should have been equipped with a flat-bed lorry.

He had to climb into the vehicle in order to guide the panels in, but a shutter slipped and crushed his leg.

Andrew Kingscott, an inspector at the HSE, said this case highlights the consequences of failing to assess risks and not planning work properly.

"What initially appeared to be a simple job ended in tragedy" he commented.

Mr Kingscott added that employers have a duty of care towards their staff, which includes equipping them with items such as safety helmets and hi vis clothing where appropriate.

This comes shortly after a food processing plant employee was injured when cleaning machinery.
