Freight company prosecuted over employee death The Health and Safety Executive (HSE) has warned employers in the freight industry to ensure that all workplace requirements are met - and that basic maintenance issues are not ignored.

This latest warning comes after a freight company was fined and prosecuted after an employee was crushed and killed by a falling metal gate on the company's site.

HSE inspector Rachel Bradshaw said: "Basic maintenance of equipment is often ignored by employers and lives are put at risk as a result. There was a very obvious defect on this gate that a simple visual check would have identified. A few moments work and simple modifications would have prevented this tragedy from occurring."

A total of 241 workers were fatally injured in 2006/07, an increase of 24 on the previous year, with the agriculture and construction sectors reporting the highest injury levels.
